U.S. Customs Invoice (by Manifested Quantity) (2 pages - posted 2018-04-06)

The U.S. Customs Invoice. It requires the shipper's name, consignee's name, IRS#, description of the goods, value of the goods, and the country of manufacture. Please indicate the U.S. Customs Port of Entry. Calculated by manifested quantity.

USMCA / CUSMA / T-MEC Certification of Origin

The USMCA Certification of Origin is strictly for eligible goods from Canada, the U.S. and/or Mexico valued over $2,500 USD. It is used to request a lower duty rate for eligible commercial goods entering the U.S. under the new Free Trade Agreement between North America. If eligible, it's important to note that this program replaces the previous NAFTA. It remains non-mandatory for your imports, and assistance is still highly recommended. If you do not wish to use our template, you may use this guide to providing the required data elements.

U.S. Certificate of Registration (CBP Form 4455 - Posted August 17, 2009)

This form is used to register goods before they leave the U.S., if intended on returning the goods after use in the foreign country.

This form is used for textile products made of fabric/yarns from one country.

U.S. Multiple Country Declaration

This form is used for textile products made of fabric/yarns from more than one country.

U.S. Radiation Declaration (FDA Form 2877)

This form is required for any product that emits radiation.
