Our US Customs forms are handled by our US Customs Agent, Pacific Customs Brokers Inc. (www.pcbusa.com). You can access the following US documents directly here.

	US Pro Forma Invoice This form can be used in lieu of a standard commercial invoice. It requires the shipper's name, consignee's name, IRS#, description of the goods, value of the goods, and the country of manufacture.
	US Customs Invoice The US Customs Invoice. It requires the shipper's name, consignee's name, IRS#, description of the goods, value of the goods, and the country of manufacture. Please indicate the U.S. Customs Port of Entry.
	US NAFTA Certificate This form is to be completed by the manufacturer of the goods or by someone who has knowledge that the goods qualify under the NAFTA Rules. The form must also be signed by an officer of the company making the declaration.
	FCC Form This form is required for electronic goods being imported into the United States. If a product conforms to FCC, it will have a FCC number on the back of the product.
	US Radiation Decalaration This form is required for any product that emits radiation.
	US Certificate of Registration This form is used to register goods before they leave the U.S., if intended on returning the goods after use in the foriegn country.
	US Single Country Declaration This form is used for textile products made of fabric/yarns from one country.
	US Multiple Country Declaration This form is used for textile products made of fabric/yarns from more than one country.
